New Patient Registration

We accept new patients who live within our practice area.  If you would like to register at the practice, please present with two forms of utility proof and if you have a passport for proof of identification please bring this along, this will speed up the registration process.

Once you have registered you will be given an appointment with a Nurse or Health Care Assistant for a routine new patient health check. This will involve a discussion around any health problems you may have and medication you may be currently taking. Your height and weight will be recorded and we will also test a small sample of urine.

Once this check is carried out, you may book an appointment to see a doctor/nurse of your choice.

You will not be registered with the practice until this process is completed.
